Im using Angry IP scan to see what IPs are alive
Then i NMAP them
I get a result Like this

Some Program or something to hook up to the port?
Well there isn't a "special" program for all ports, all though there are "commoners" for all many of them. For example
FTP almost always runs on 21
HTTP on 80
SMTP 25
blablablabla etc...
like if you scan an IP address and it finds that something is listening to the port 21 (open) you could connect to it using an FTP client (if it's an FTP server that is listening), of course it almost always has a password, but sometime it has anonymous logins which can be used as an advantage. Or if the machine has something listening you could try using it to send mail if it doesn't have any authentication on it. Sometimes if it's a website you are scanning and it has port 25 open you could try goto http://mail.domainame.com/org/se/nl...etc and it may have a weblogin for the mailserver.
Anyway, to connect to a port you would need a program that "speaks the same language" as the program listening for connections.
